What to do before a visit:

Print the Confirmation Number

Bring Confirmation # to appointment

New Aviators must create an account

A NEW 8500-8 form must be completed for every exam

Basic Medical

Hydrate
-
Avoid excess caffeine, salt, & sugar
-
Bring glasses or wear contacts if needed​
NO Color altering glasses
NO X-chrome contacts

Bring government issued photo ID
-
Bring PI # and FAA documentation for Special Issuance, SODA, or CACI (If applicable)
